This study assessed the impact of a psychoeducational intervention on family dynamics for couples with children diagnosed with ADHD. Parents participated in 5 weekly sessions that provided information on ADHD and taught cognitive-behavioral strategies and behavior modification techniques. Questionnaires assessed ADHD symptoms, family conflict, and social difficulties before and after the intervention. Results showed decreases in ADHD symptoms, family conflict, and social problems. Parents reported richer relationships with their children and better understanding of their behavior. The psychoeducation program was found to improve family dynamics for those with children diagnosed with ADHD.

The study used data from over 5,700 Norwegian adolescents and their parents, collected over 10 years. Structural equation modeling found that parental anxiety/depression predicted adolescent anxiety/depression, and these associations were mediated by current parental symptoms and adolescent self-esteem. Physical activity moderated the association between maternal anxiety/depression and adolescent symptoms. Overall, the findings suggest familial aggregation of anxiety/depression over 10 years and that adolescent self-esteem and physical activity may influence this transmission.

Abstract: Parents of children with Autism Spectrum Disorder (ASD), a life-long developmental disorder, responded to an online survey considering their stress experience, affective symptoms and marital satisfaction. As these parents sourced different programs for their children, type of program was used to assign parents to different groups in order to consider their stress, affective symptoms and marital satisfaction. The type of programs parents used included the Applied Behaviour Analysis (n=15); Early Intervention Centre (n=13) and no formal program (n=16). Parents of children with ASD in the ABA group reported significantly lower parental stress scores, lower affective symptoms scores and higher marital satisfaction scores compared to the other two groups. Abstract: Parents of children with Autism Spectrum Disorder (ASD), a life-long developmental disorder, responded to an online survey considering their stress experience, affective symptoms and marital satisfaction. As these parents sourced different programs for their children, type of program was used to assign parents to different groups in order to consider their stress, affective symptoms and marital satisfaction. The type of programs parents used included the Applied Behaviour Analysis (n=15); Early Intervention Centre (n=13) and no formal program (n=16). Parents of children with ASD in the ABA group reported significantly lower parental stress scores, lower affective symptoms scores and higher marital satisfaction scores compared to the other two groups. IMPORTANCE Anxiety is common among youth with autism spectrum disorder (ASD), often
interfering with adaptive functioning. Psychological therapies are commonly used to treat
school-aged youth with ASD; their efficacy has not been established.
OBJECTIVE To compare the relative efficacy of 2 cognitive behavioral therapy (CBT) programs
and treatment as usual (TAU) to assess treatment outcomes on maladaptive and interfering
anxiety in children with ASD. The secondary objectives were to assess treatment outcomes
on positive response, ASD symptom severity, and anxiety-associated adaptive functioning.
DESIGN, SETTING, AND PARTICIPANTS This randomized clinical trial began recruitment in
April 2014 at 3 universities in US cities. A volunteer sample of children (7-13 years) with ASD
and maladaptive and interfering anxiety was randomized to standard-of-practice CBT,
CBT adapted for ASD, or TAU. Independent evaluators were blinded to groupings. Data
were collected through January 2017 and analyzed from December 2018 to February 2019.
INTERVENTIONS The main features of standard-of-practice CBT were affect recognition,
reappraisal, modeling/rehearsal, in vivo exposure tasks, and reinforcement. The CBT
intervention adapted for ASD was similar but also addressed social communication and
self-regulation challenges with perspective-taking training and behavior-analytic techniques.
MAIN OUTCOMES AND MEASURES The primary outcome measure per a priori hypotheses was
the Pediatric Anxiety Rating Scale. Secondary outcomes included treatment response on the
Clinical Global Impressions–Improvement scale and checklist measures.
RESULTS Of 214 children initially enrolled, 167 were randomized, 145 completed treatment,
and 22 discontinued participation. Those who were not randomized failed to meet eligibility
criteria (eg, confirmed ASD). There was no significant difference in discontinuation rates
across conditions. Randomized children had a mean (SD) age of 9.9 (1.8) years; 34 were
female (20.5%). The CBT program adapted for ASD outperformed standard-of-practice CBT
(mean [SD] Pediatric Anxiety Rating Scale score, 2.13 [0.91] [95% CI, 1.91-2.36] vs 2.43 [0.70]
[95% CI, 2.25-2.62]; P = .04) and TAU (2.93 [0.59] [95% CI, 2.63-3.22]; P < .001). The CBT
adapted for ASD also outperformed standard-of-practice CBT and TAU on parent-reported
scales of internalizing symptoms (estimated group mean differences: adapted vs

Fallo primario del injerto
Este documento resume los resultados de un estudio sobre los factores de riesgo asociados al fallo primario (precoz) del injerto tras un trasplante cardíaco. Algunos de los hallazgos principales son:

Today more than ever, hepatocellular carcinoma therapy is experiencing profound and substantial changes.
The association atezolizumab (ATEZO) plus bevacizumab (BEVA) has demonstrated its effectiveness in the post-operative treatment of patients, improving the results that can be achieved with liver resections. This after the failure of the use of sorafenib in the already historic STORM study.
On the other hand, the prognostic classification of BCLC is now widely questioned. It is now well recognized that the indications for surgery for patients with hepatocellular carcinoma are certainly narrow in BCLC and no longer reflect what is common everyday clinical practice.

Belongingness in Medicine - ACGME Equity Matters Webinar - June 2024
We presented this webinar to the ACGME Equity Matters Alumni on June 27, 2024. We defined belonginess and reviewed two theoretical frameworks for belongingness. We discussed the reasons for conducting research about belongingness in medicine and presented findings from our study. "Belongingness among first-generation medical students at Stanford School of Medicine" (MedEdPublish, 2023, Delgado et al.) We then led the audience in small group discussions about potential interventions to improve trainee belongingness by medical schools and residency programs

The uroculturome indicates the profile of culturable microbes inhabiting the urinary tract, and it is often required to do a urine culture to find an effective antimicrobial to treat UTIs. This study targeted to understand the profile of culturable pathogens in the urine of apparently healthy (128) and humans with clinical UTIs (161). In urine samples from UTI cases, microbial counts were 1.2×104 ± 6.02×103 colony-forming units (cfu)/ mL, while in urine samples from apparently healthy humans, the average count was 3.33± 1.34×103 cfu/ mL. In eight samples (six from UTI cases and two from apparently healthy people) of urine, Candida (C. albicans 3, C. catenulata 1, C. krusei 1, C. tropicalis 1, C. parapsiplosis 1, C. gulliermondii 1) and Rhizopus species (1) were detected. Candida krusei was detected only in a single urine sample from a healthy person and C. albicans was detected both in urine of healthy and clinical UTI cases. Fungal strains were always detected with one or more types of bacteria. Gram-positive bacteria were more commonly (OR, 1.98; CI99, 1.01-3.87) detected in urine samples of apparently healthy humans, and Gram -ve bacteria (OR, 2.74; CI99, 1.44-5.23) in urines of UTI cases. From urine samples of 161 UTI cases, a total of 90 different types of microbes were detected and, 73 samples had only a single type of bacteria. In contrast, 49, 29, 3, 4, 1, and 2 samples had 2, 3, 4, 5, 6 and 7 types of bacteria, respectively. The most common bacteria detected in urine of UTI cases was Escherichia coli detected in 52 samples, in 20 cases as the single type of bacteria, other 34 types of bacteria were detected in pure form in 53 cases. From 128 urine samples of apparently healthy people, 88 types of microbes were detected either singly or in association with others, from 64 urine samples only a single type of bacteria was detected while 34, 13, 3, 11, 2 and 1 samples yielded 2, 3, 4, 5, 6 and seven types of microbes, respectively. In the urine of apparently healthy humans too, E. coli was the most common bacteria, detected in pure culture from 10 samples followed by Staphylococcus haemolyticus (9), S. intermedius (5), and S. aureus (5), and similar types of bacteria also dominated in cases of mixed occurrence, E. coli was detected in 26, S. aureus in 22 and S. haemolyticus in 19 urine samples, respectively. Gram +ve bacteria isolated from urine samples' irrespective of health status were more often (p, <0.01) resistant than Gram -ve bacteria to ajowan oil, holy basil oil, cinnamaldehyde, and cinnamon oil, but more susceptible to sandalwood oil (p, <0.01). However, for antibiotics, Gram +ve were more often susceptible than Gram -ve bacteria to cephalosporins, doxycycline, and nitrofurantoin. The study concludes that to understand the role of good and bad bacteria in the urinary tract microbiome more targeted studies are needed to discern the isolates at the pathotype level.

INTRODUCTION RESULTS The Inattention subscale (p=0.012), the Total subscale (p=0.003), and the Oppositional Defiant Disorder (ODD) subscale (p=0.03) of the The DuPaul Questionnaire showed a significant decrease from T1 to T2 (Fig. 1). Such decrease was also found for the Conflict subcale (p=0.013) of the FES scale (Fig. 2), as well as for the Classmate Problems subscale (p=0.002), and the Difficult Total subscale (p= 0.003) of the SDQ (Fig. 3). In the evaluation questionnaire, parents reported high satisfaction with the objectives of the psychoeducation. They referred a richer relationship with their children, and a better understanding of their behaviour. We designed an experimental study to help parents manage the behaviour of their ADHD children. We administered psychoeducation to 12 couples with ADHD children divided into two modules of five weekly sessions of 45 minutes, with the following structure: MODULE 1 Session 1 . Information about ADHD MODULE 2 Based on cognitive-behavioral strategies and behavior modification : Session 2 . Definition of behavior and techniques for enhancing behavior (I): the reinforcement Session 3 . Token economy Session 4 . Techniques to reduce or eliminate behavior (II): negative consequences (time-out) and extinction Session 5 . Training in problem solving. Couples filled in the DuPaul Questionnaire, the Strengths and Difficulties Questionnaire (SDQ), and the Family Environment Scale (FES) at two times, before (T1) and after (T2) psychoeducation. At the end of the program, parents also completed an evaluation questionnaire. MATERIALS AND METHODS From the data gathered, we can suggest that psychoeducation reduces family conflict, helps parents have a better management of their children’s behaviour, and reinforces the relationship between parents and children. Therefore, we can conclude that a five-session psychoeducation program may improve family dynamics.
