Precision medicine models for treating depression, managing addictions and achieving sustained behavior change are largely outside of current clinical practice. Yet, changing self-regulatory behavior is fundamental to the self-management of complex lifestyle-related chronic conditions such as depression and substance use disorder - two top contributors to the global burden of disease and disability.
To optimize treatments and address these burdens, methods to facilitate behavior change and self-regulation must be better understood in relation to their neurobiological underpinnings. Treatment strategies can then be developed that leverage the recent advances in immersive environments, machine learning, and wearable sensing technology and apply them to treat depression, manage addictions, and facilitate behavior change using a precision medicine model that is personalized to the individual.
This presentation will review the conceptual framework and protocol for a large multi-subject longitudinal study named Project ENGAGE. The ENGAGE study integrates neuroscience with behavioral science to better understand the self-regulation related mechanisms of behavior change for improving mood and weight outcomes among adults with comorbid depression and obesity. We collect assays of three self-regulation targets (emotion, cognition, and self-reflection) in multiple settings: neuroimaging and behavioral lab-based measures, virtual reality, and passive smartphone sampling.
By connecting human neuroscience and behavioral science in this manner within the ENGAGE study, we can develop a prototype for elucidating the underlying self-regulation mechanisms of behavior change outcomes and their application in optimizing intervention strategies for multiple chronic diseases.

Orphans in the Desert is an online platform that aims to help people with rare diseases by allowing them to input unstructured health data to be analyzed by IBM's Watson for potential diagnoses, and to cluster patients together in order to incentivize treatment research. It serves as a safe place for rare disease patients to describe their symptoms freely without bias or preconceptions, and provides educational resources, support groups, and information on clinical studies. The platform allows for both individual analysis of user data as well as macro-level statistical analysis of particular or general rare diseases.
ALIVIO is developing a mental health app called MHA that connects to EEG devices to measure brain activity and mental health conditions. The app will allow users to (1) do self-assessments of their mental condition, (2) record measurements and analyze results to determine their mental state, and (3) connect to and share data with healthcare providers if needed. For healthcare providers, the app provides access to patient data to assess mental health before appointments and allows constant monitoring of patient data to recommend treatment plans. The app collects daily mental activity data from brain stimulation devices that can be shared with healthcare providers like psychiatrists and neurologists for consultation and medical help.

Future work includes using these methods for Alzheimer's and assessing driving ability in pre-Alzheimer's patients.

Areas seeing rapid adoption due to COVID-19 include remote rehabilitation, periodic testing, environmental sensing, behavior tracking, centralized records, data-driven processes, personalized medicine, autonomous therapy, resource triage, telemedicine, continuity of care, and wearable therapeutics. Future work includes using these methods for Alzheimer's and assessing driving ability in pre-Alzheimer's patients.
